Since its establishment, the academic and health promotion activities of the Institute of Child Health have included educational programs in the field of child health, research activities directed towards the solution of problems related to child health and provision of health in various fields of pediatrics, with emphasis on preventive aspects.
The Institute is an academic body within the University of Istanbul and was founded in accordance with the Higher Education Law passed in 1982. Since its establishment, the Institute has shared the same premises as the Pediatric Department of the Istanbul Faculty of Medicine in Çapa and the two institutions have worked jointly in running the inpatient and outpatient services, the laboratories, lecture halls, the library and other services of the Children's Hospital. Since 1994, the Institute of Child Health is also working in close cooperation with the Gyn/Obs Department of the Istanbul Faculty of Medicine in conducting the educational, research and health provision activities of the Woman and Child Health Education and Research Unit. The Institute staff belong to various disciplines including pediatrics and pediatric subspecialties, public health, psychiatry, demography, sociology, anthropology, pedagogy, nutrition, biostatistics, biochemistry, molecular genetics and physiotherapy. The majority of the Institute staff are University of Istanbul employees and this group includes 12 professors, 7 associate professors, 6 specialists, 2 biologists, 1 antropologist 18 residents, 9 nurses, 1 administrative secretary, 6 secretaries/auxiliary staff, 1 dietitian, 2 physiotherapists, 1 chemists, 3 nurses aides.

EDUCATIONAL ACTIVITIES
Resident Training in Pediatrics
To date 71 pediatricians have completed their training under this program which was started in 1984. At present, there are 6 resident have completed their training but have not yet received their degree. No new resident was admitted to this program since it had been discontinued in 1998 The program has been restarted since September 2005 and at present there are 18 new residents in training.
Master's Degree Programs
These programs aim to train medical and non-medical staff who will work in pediatric centers or related institutions offering pediatric health services.

To date 142 Master's Degree students have successfully completed these programs. At present, there are 5 students enrolled in these programs.
DOCTORATE PROGRAMS
To date, Doctorate programs have been initiated in the fields of Medical Genetics and Social Pediatrics.
6 students have successfully completed their doctorate programs. At present, 2 student are enrolled in Social Pediatrics.
CERTIFICATE PROGRAMS
The Family Health Department of the Institute since 1985, has been organizing "workshop" type courses running for one or two weeks for the in - service training of health professionals of different levels. These programs mainly aime to enhance the performance of the health workers in communication skills, approach to problems, health education issues and educational methods.
In the period between 1985-1989, health workers from Istanbul and from other regions in Turkey and including doctors, nurses, midwives, psychologists, health educators have benefited from these programs. Many of these programs were sponsored by UNICEF.
In 1990, only teachers in nursing schools responsible for the teaching/training in child health and obstetrics were admitted to these certificate programs, conducted jointly with the General Directorate of Health Education of the Ministry of Health and UNICEF. These "teacher training" programs were also realized in Kayseri and in Erzurum and were attended by 84 nurse teachers.
The Institute also organized short postgraduate courses addressing pediatricians and other professional groups such as pharmacists. These programsa covered topics such as diarrhea and oral rehydration and acute respiratory infections in children. In 1991, upon the request of the Istanbul Association of Pharmacists, the Institute cooperated with this Association and family planning issues were included in the in-service training programs of the pharmacists.
Since 1994, certificate programs have been conducted by the team of the Woman/Child Education and Research Unit.
ACTIVITIES OF THE DIFFERENT UNITS OF THE INSTITUTE
Woman / Chlid Education and Research Unit
This Unit, situated in the Istanbul Faculty of Medicine Hospitals area in Capa, was established in 1994 by the Family Health Department of the Institute jointly with the Gyn/Obs Department of the Istanbul Faculty of Medicine, as a three year project of the Human Resource Foundation financed by the European Union. In this Unit, the Family Health Department of the Institute offers integrated health services to families in pregnancy prevention and pregnancy termination upon request, and also informative services in woman health and infant care, and counseling services in all these areas. The Unit also conducts research activities in sexual health, infant care and related issues. The Unit is also the site of educational programs on reproductive health and related subjects, organized and conducted in collaboration with other University departments. In addition to University staff, the Child Health Association, a non – governmental organization, is supporting these programs by providing additional staff for the educational as well as for the research activities of the Unit. In the interactive educational programs of the Unit, in addition to clinical knowledge and skills, counseling and educational skills are also addressed. Special programs in teaching methodology have been organized for medical teachers, in conjunction with the Center for Medical Education of the Istanbul Faculty of Medicine
This Unit has offered an integrated health service to over 15.000 families (first visits) between the years 1994 and 2002. More than 500 professionals including doctors, pharmacists, nurses and educators have attended the training of trainers courses offered by the Unit. Twenty four of these participants were professionals from the Turki countries. The Unit, supported by other institutions initially, has received great support from the University of Istanbul in recent years and has gradually become less dependent on other funds.
Medical Genetic Unit
This Unit has been established within the Child Health Institute in 1985. Since then, a Clinical Genetics Outpatient and a Cytogenetic Laboratory have been in service for a large population. In 1989, a Prenatal Diagnosis and Research Center was established and has served as a Pioneer in prenatal testing in Turkey.
The services provided by the unit include clinical genetics and genetic councelling services, cytogenetic and molecular genetic studies. These services are being provided by a team of experts and to date, 30.000 families have been evaluated by this group. In addition to routine testing, moleculer cytogenetic (FISH and Telomeric FISH) studies are also performed in the Cytoenetics Laboratory is accepted as a reference center for Turkey. Approximately 15.000 postnatal and 12.000 prenatal tests totaling ~27.000 karyotype analyses have been performed to date in this laboratory. In the years 2004-2005, fibroblast cultures in 57 patients and peripheral blood studies in 745 patients were conducted for postnatal diagnostic purposes. Analyses were conducted on 1054 amnotic fluid, 140 chorionic villus tissue, and 138 fetal blood samples. 540 molecular cytognetic analyses were performed.
The Molecular Genetics Laboratory was established in 1988 and since 1999 provides routine testing for: Fragile-X syndrome, Long QT syndrome, non syndromic deafness, (35 del G mutation screening in the Connexin 26 gene), autosomal dominant spastic paraplegia, GMZ activator deficiency, Carvazal syndrome, Norrie Disease, Waardenburg syndrome, CADASIL syndrome and alkaptonuria. Ongoing research subjects include cloning of new genes for cardiac arrhythmias, defining new genes for hereditary deafness, and cloning of new gnees in hereditary cardiac diseases.
In the Medical Genetics section 21 Master’s 8 PhD students have already completed their academic education and presently 4 master’s and 4 PhD students are working towards their degree. In a 16 years period, 72 articles in SCI indexed and 72 in national journals have been published and many oral/written presentations have been given in international and national congresses.
Trace elements and gastroenetrology
The trace elements laboratory is, in addition to the Genetics Laboratory, another specialized unit of the Institute which conducts research and contributes to service provision. The Trace Element laboratory was founded in 1973 supported by government research funds and became a Unit of the Nutrition and Metabolism section of the Institute in 1982. Initially, this laboratory was involved in studies on chromium which received international acclaim. Professor G Saner is the editor of the book entitled “Chromium in Nutrition and Disease”, Published by Alan R Liss Co, NY, in 1980. Professor Saner is also the author of a chapter in “Manganese in Health and Disease” (ed. Klimis – Tavantzis DJ) published by CRC Pres, Inc., in 1994.
At present serum levels for zinc, copper and lead; levels in hair samples for zinc, copper, lead, manganese, and cadmium; liver tissue levels for copper are determined in this laboratory for diagnostic and also research purposes.
In 1989, an extension was made to the Trace Element Laboratory by the institution of a Pediatric Gastroenterology Laboratory. The Following determinations are rountinely conducted in this laboratory: lipoprotein X in blood; fat, starch, pH, occult blood, chymotripisinogen, steatocrit, and alfa-1 antitripsin in stool; ande reducing substances in both urıine and stool.
Research Projects conducted in this laboratory:
· Adequacy of breast feeding in the first 6 months of life. 1983-1985 – Supported by TUBİTAK (Govt. Institution for scientific and research studies)
· Serum zinc and copper levels in septisemic infants. 1994-1997 – Supported by İstanbul University research funds.
· Hair and blood lead levels in healthy children followed in the well child clinic. 1998-2001. Supported by İstanbul University research funds.
· Lead levels in blood and hair and psychomotor assessment in school children in a risk environment. 1999-2001. Supported by İstanbul University research funds.
· Frequency of Helicobacter and Cag A in children with gastrointestinal symptoms. 2000-2002 supported by Istanbul University research funds.
· Blood lead levels in children aged 0-2 years. – 2006. Supported by İstanbul University research funds.
· Blood lead levels in children of school age 2006. Supported by İstanbul University research funds.
AUXOLOGY
Educational activities in this section include periodic meeting with other disciplines for discussion of pediatric endocrine patients, joint meetings with adult endocrinologists for discussion of patients being transferred to the adult endocrine clinic, and educational sessions (a total of 5 hours each month) for the parents of diabetic children . In addition, camps for diabetic children have been initiated in 1996 and are continuing to be organized twice each year (a summer camp and a winter camp)
DR. FREDERİC SHORTER COMPUTER UNIT
Dr. Frederic Shorter served as a Faculty member of the Institute in 1993-1994 and his name was given to this Unit which was established in 1994. This Unit serves as a data processing center for the Children’s Hospital inpatient and outpatient data and for various research projects.
OTHER ACTIVITIES
Congresses/Meetings:
The Institute, jointly with the Department of Pediatrics of the Istanbul Faculty of Medicine and the National Pediatric Association, organizes Pediatrics Days every year. These meetings which address pediatricians, focus on one special topic each year and the programs include lectures, panel discussions and free communications.
Lectures:
Each year in memory of the late Professor Cihad Gürson the Institute organizes the "Gürson Lecture" given by an invited renown pediatrician.
Outpatient Clinic and Other Clinical Service Activities:
These activities are conducted jointly by the ınstitute staff of the Pediatric Department of the İstanbul Faculty of Medicine,
· Emergency Pediatrics: In 2005, 7300 patients were admitted to this Unit. For screening. Of these, 400 were admitted to the Pediatric Intensive Unit and 450 to the Pediatric Emergency ward.
· General Pediatrics Outpatient Clinic: In 2005, 27132 infants/children presented to this Clinic
· Well Child Clinic: The number of infants/children presenting to this Clinic was 5119 in 2005.
· In 2005, 925 new and 3675 old patients presented to this clinic. Of these 236 were hoapitalized in the Endocrine Unit.
· In 2005, 2809 patients presented to the Gastroenterology clinic (The Nutrition and Metabolism Unit of the Institute Works jointly with The Gastroenterology Unit). Of these, 179 were hoapitalized.
· In the pediatric neurology clinic, conducted by the Pediatric Neurology Unit jointly with the Faculty of Medicine staff, 3029 new patients and 6069 follow-up patients were seen in 2005. Of these, 142 were hospitalized.
