If you are interested in becoming a patient:

Contact us directly by telephone. Call the IUFD Reception Desk at +90 212 4142599 and tell the receptionist you are interested in becoming a IUFD patient. You will be asked for some general information.

Bring all x-rays from the last five years.

Complete the enclosed health questionnaire and patient information sheet.

Bring ALL current medications with you.

Bring all insurance information, your insurance card, and photo identification.

At your first visit, a staff dentist will examine you to determine the difficulty of your case in our oral diagnosis department.

There is no charge for this screening appointment, but you will need a current set of x-rays.

If you are under the care of a physician, please bring the name and address of your physician.

All our patients have a choice of having their care provided by a student dentist under the supervision of qualified dentist and academic staff, a resident dentist or one of our world-renowned faculty members.
